All formalities for admission must be completed between 11.10.2010 to 13.10.2010 from 10.00 AM to 1.30 PM, Thereafter, admission will not be considered under any circumstances. 1. Fee Book to be collected from the Junior Section office and Fees to be deposited in Cash (in favour of Mahadevi Birla Girls' Higher Secondary School) in the UNITED BANK OF INDIA, Darga Road Branch, prior to filling up of Admission Form. Payment by cheque for new admission will not be accepted. 2. Filling up of Admission Form should be completed by 13.10.2010 in the Junior Section building. Both the parents must be present to sign the Admission Form. The following documents are to be submitted at the time of admission: 1. A photocopy of the Birth Certificate (to be attached with the Admission Form and the original to be brought for verification) 2. A recent coloured passport size individual photograph (size 3.5 cm × 2.5 cm) of the child and both the parents. (to be affixed on Admission Form) 3. Submit document in case of SC/ST/OBC/MINORITY. 4. SCHOOL BUS : Bus facility will depend on availability of seat in the particular area. 5. Fee once deposited will not be refunded. Full set of Text books / Exercise books and stationery of Upper Infant will be available in the Junior Section Assembly Hall on the 21st & 22nd of March’2011 from 10.00 AM - 2.00 PM . The cost of the same will be Rs. 1000/- approximately. 6. IDENTITY CARD : Parents must get the school uniform from the dress supplier, as given in the prospectus, by the 15th of January’2011. They should submit the Identity Card form duly filled with a photograph of their child in school uniform to the Junior Section office by 15th February’2011. 7.FEE TO BE DEPOSITED AS STATED :-
